【コラム】ホメオパシーとは何か。その効果は。

アレルギー関係のブログなどを拝見していると、ホメオパシーというものについてのものをよく見かけるようになりました。

私もはじめてきく言葉でしたが、どうも、その効果などについては議論があるようです。

ウィキペディアで調べてみましたが、やはり微妙な感じです。ブログを拝見していても、これでよくなっている場合もあれば、苦しみながら続けている方、効果が現れないと言われている方など、差があるようです。

個人の体質との相性、処方する方（というのかわかりませんが）の技能・能力などによるのかもしれません。

ちょっと気になる現象でしたので、ここではウィキペディアの記事を引用して起きたいと思います。

ホメオパシー

ホメオパシー（homeopathy）は「同種療法、同毒療法、同病療法」と訳される。また、ホーリスティック医療に分類される療法のひとつである。日本においては代替医療のひとつに数えられる。

ホメオパシー（同種療法）とは、

  • ある症状を持つ患者に、
  • もし健康な人間に与えたら、その症状と似た症状を起こす物質を
  • きわめて薄くしてわずかに与える

ことによって、症状を軽減したり治したりしようとする療法のことである。たとえば、解熱を促そうとする時には、健康な人間に与えたら体温を上げるような物質を含む物質を患者に与える。このことによって、極めて短時間発熱が促進されるが、すぐに解熱に向かうとされている。

概要

2006年現在、ホメオパシーが有効であるという根拠を科学的に示すことは成功されておらず、実際の治療効果について多くの科学者・医者が疑問視している。このように、非科学性を問題とする立場からは、疑似科学と考えられ偽医療の一つに過ぎないとして扱われることも多い。一方で欧米では、多くの療法家が手掛けているもっとも広く普及した代替療法の一つである。なお、同種療法と代替療法は似ている言葉だが、意味はまったく異なる。ホメオパシーは、日本においては医療として認められていないため、代替療法と呼ばれるのである。大別してクラシカルとプラクティカルの2派があり、前者は理論を重視しその時々の症状よりも患者個人の性質に重きを置くのに対し、後者は臨床を重視しその時々の症状に注目する。

歴史

ドイツ人医師ザームエル・(クリスティアン・フリードリヒ)・ハーネマン(Samuel Christian Friedrich Hahnemann, 1755年 - 1843年)によって始められた。ハーネマンの主著『オルガノン』(1810年刊)によると、同様なものは同様なものを治すという類似の法則があり、ある物質を健康な人に投与した時に起こる症状を治す薬としてその物質が有効であると主張。その物質が限りなく薄く希釈される(ハーネマンの表現を借りれば「物質的でなくなる」)ほど、霊的な治癒能力を得ることが出来ると考えた。

ハーネマンの死後、ホメオパシーの流れは分裂した。「低効能派」は希釈度合を濃くして患者に投薬し薬効を期待する一方で、「原理派」は気の概念を援用してあくまでハーネマンの主張通りの薬効を主張している。

理論

ホメオパシーでは、一般的に小さな砂糖粒に物質を溶かしたさまざまなレメディ(主なレメディの一覧：英語) と呼ばれるものを服用する。レメディの種類は2000種とももっと多いとも言われる。その中で、応用範囲が広く、常用されるのは40種くらいである。レメ ディをすでに現れている症状の治療目的に使われることもあるが、本格的な治療に当たっては、症状の治療よりも、その病気を起こさせた根本の原因を治療しよ うとする。このために、ホメオパスと呼ばれるホメオパシー治療を専門に行う者の処方によりレメディを服用する。

レメディは、基本的に体にとっての毒物を非常に少量含む。この毒物に対する体の抵抗を意図的に起こすことにより、自己治癒力を含む生命力を高め、肉 体的、心理的、精神的な方向が本来あるべき方向へ修正されると言われる。これを「微量の法則」と呼ぶ。しかし、錠剤中、または水溶液中に、1分子たりとも 有効成分であるとされる毒物が含まれないほど希釈されていることが多々あり、この点がホメオパシーが疑似科学または偽科学であるとする論のひとつの根拠と なっている。この科学的疑問点については、後に詳述する。

ホメオパスは人が健康なら体も健康という基本的な考えの元に働きかけ、心理的、感情的、精神的なゆがみを補正するようにレメディを処方する。このた め、ホメオパスとのセッション（面会）では、十分な時間(1時間程度の事が多い)をかけ、患者の心理的、精神的な状態や、成長の過程、とくに過去の大きな 問題についてのインタビューが持たれる。そうして基本的な人のタイプを見て、現在の問題を判断しレメディが処方される。

レメディ (療剤)

ホメオパシーに用いるレメディ (療剤) は、地上におけるさまざまな物質から成分を取り出して、10倍ないし100倍の希釈を行い、それを震盪（よく振ること）する。この希釈・震盪を6回から1 万回繰り返して、最後にこれを小さな砂糖粒に染み込ませて作成する。たとえば10倍希釈・震盪を9回繰り返して作ったレメディは9X（Xは10倍希釈を意 味する）、100倍希釈・震盪を30回繰り返したレメディは30C（Cは100倍希釈を意味する）と呼ばれる。もっともよく使われるのは30Cであり、ほ かに200C、1,000C（1Mと呼ぶ）、10,000C（10M）、6Xなどが用いられる。

希釈のため、原成分はレメディーの中には極めてわずかしか含まれない。特に高度に希釈した場合には、当然に、計算上、一粒のレメディに原成分が1分 子たりとも含まれないこととなる。しかし、より希釈・震盪したものの方が、より効果が高く、また人間の精神面などより中心的な部分に作用すると考えられて いる。これは、希釈・震盪によって、希釈液が原液の治癒エネルギーに出会うことにより、希釈液のエネルギーに変化が生じて治癒エネルギーを持つようになる ため、というように説明される。従って、レメディの中に原成分が含まれる必要はないのだという。

原料となる物質は、鉱物、植物、動物などであるが、特に初期に開発された物には、伝統的な薬草も多い。

科学的疑問点

ホメオパシーの問題点はその有効性が科学的（統計的）に立証されていないことである。

ホメオパシーが拠り所とする「少量の毒によって健康を増進する」という考え方は、アレルギー治療における減感作療法と類似したものと捉えられることもあるが、そもそも与えている物質がアレルゲンのように症状の原因ではないという点で減感作療法とは異なっている。

また、減感作療法についてはそれが有効であるということが科学的にも立証されており、またその効果は免疫に寛容を誘導することにより発揮されているというメカニズムも解明されている。しかし、ホメオパシーの理論ではさらに低濃度の物質を用いるため、この点においても減感作療法とは異なる。

過度の希釈に関しては、たとえば、最も広く利用される30Cの希釈(10030倍希釈、すなわち1060倍希釈)を行った場合、1分子の原成分を含むためには1060分 子の水、実に約30,000,000,000,000,000,000,000,000,000,000,000リットルの水(300兆リットルの1兆倍 の1兆倍;地球に換算すると約300億個分の容積となる)が必要となるが、実際に摂取する砂糖粒は小指の爪以下の小さな塊であり、もはや原成分はまったく 存在していないはずである。これが、薬理学の常識とはかけ離れているとされるゆえんである。

また、希釈・震盪によって、原液が希釈液に「治癒エネルギー」を与えるとする説明に関しても、その治療エネルギーが具体的に何なのかが示されていな い。「水分子の構造に記憶を残す」と説明する者もいるが、液体状の水分子は流動的で熱運動によって常に情報が失われる方向へと構造が変化しており、たとえ 一時的に水分子が造る構造に情報が記憶されたとしても、次の瞬間（ピコ秒オーダー）にはその情報は失われていると物理的には解される。

これまでにホメオパシーの有効性を立証したと主張する論文が何度か発表され、そのたびに議論になったが、いずれも対照群の設定や母集団の数、主観の入りにくい調査の実施などが不十分とされ信頼性が低いとされてきた。医学専門誌Lancetの2005年8月号に、ホメオパシーに関する臨床検討の論文110報をメタ解析した調査が報告され、これにおいてもホメオパシーの効果はプラセボと同等であると結論されている。

このことを問題とする立場の者は、ホメオパシーが疑似科学であるとし、プラセボ以上の治癒効果の可能性が有る「代替医療」ではなく、そもそも全く治療効果のない「偽医療」であると主張している。

ホメオパスからは、動物や乳幼児にも有効であること、１人の患者に対して特定のレメディしか有効でないことから、プラセボではありえないという反論が出ているが、客観的事実や科学的な根拠・証拠は示されていない。

日本以外における評価

イギリス、ドイツ、フランスにおいては健康保険の適用が認められており、ドイツ、インドでは大学の医学部のカリキュラムに組み入れられ、国家の認定を受けていない者の処方は違法とされている。またアメリカの国立衛生研究所(NIH)の一部門である国立補完代替医療センター(NCCAM)においても、その有効性について、有効性の有無を含め、研究がなされている。

インドでは今も活発な伝統医学であるアーユルヴェーダの 長い伝統があるためか、一般によく受け入れられており、ホメオパシーの医師が多い。また、ホメオパシーの技術が進んだ国と考えられており、アーユルヴェー ダと同様に国家資格で治療している。治療効果は高いとされ、副作用が少ないため好む人も多い。アーユルヴェーダが病気治療よりも健康維持、健康増進を主な 働きかけとするように、ホメオパシーもまた同様の目的で利用する人が多い。
